Determine the feasibility of using retired physicians to provide specialty care as part of the network’s primary care services.

Implement evidence based patient education and screening services using health educators and community health workers.

Provide counseling, referrals, telepsychiatry and other behavioral health needs to address patient needs and existing gaps in services.

Expand access to substance abuse treatment to include age-appropriate evidence based counseling for patients including those having dual diagnoses.

Collaborate with townships and counties to expand rural public transportation for communities with large numbers of older residents and disable patients.

Train and implement patient navigators who will assist community clinic patients and others in locating medical, dental, and specialty services.

Determine the incidence of diabetes and other chronic diseases in the network’s service area in collaboration with its network and university partners.

Initiate efforts to address high rates of suicide in Northern Wisconsin and Upper Michigan to include veterans and vulnerable youth.

Within five years, implement a full range of behavioral health services for patients with dual diagnoses

Complete an assessment of the accessibility of medication assisted treatment (MAT) programs in its service area and develop services if appropriate.

Determine how best to expand access to certified and cost effective childcare services for low income and working families

Work with border communities of Michigan and Wisconsin to increase the availability of emergency medical services including mutual aid.

Address issues of maternity care and women health to address a lack of local delivery services and recruitment of women’s health specialist (ob/gyn).

Expand annual physicals for older residents to include ongoing health screening for cancer, heart disease, diabetes, and other chronic diseases.

Expand respite and in-home care for seniors with Dementia, Alzheimer’s, and others debilitating illness to include training of care providers and families.

Establish a health professions recruitment program for hospitals and other providers to improve access to care.

Establish an age-friendly community for the large population of older residents living in Northern Wisconsin and Upper Michigan.
